Diabetes Education, Support and Management

Our Diabetes Care Team offers individual and group education with flexible hours at multiple locations in Elgin County. All adults living with diabetes or prediabetes, and those at risk for diabetes are invited to access these free services.

We can help you:

  • Understand insulin and medication
  • Plan healthy meals and snacks
  • Monitor your blood pressure
  • Read food labels
  • Manage your weight
  • Be more active
  • Learn to care for your feet

  • Stay up to date on new products and services

When it comes to managing your diabetes and maintaining health and wellbeing, our nurse and dietitian team is here to support you. We provide in-person, phone, or email appointments, and updates to your primary care provider. We also offer grocery store tours and classes on topics like healthy eating, lifestyle tips, and preventing diabetes complications. Programs vary by site. Our goal is to help you live well with diabetes through care that fits your lifestyle.
