First Five West Elgin

The First Five West Elgin program supports children under the age of 6 who do not have a primary care physician or nurse practitioner.  The First Five program strives to increase access to health promotion and preventative health care services.

Eligibility:

• Child is under the age of 6; AND
• Does not have a family doctor or nurse practitioner; OR
• Has a family doctor or nurse practitioner outside of the region (ex. GTA);
• Ontario Health Card is not required
• Seeking a Primary Care Provider Health Care Connect

The First Five Nurse Practitioner and Nurse can provide the following services:

• Well Baby/Child Assessments
• Immunizations
• Developmental Screening
• Nutrition Support
• Referrals
• Assistance with navigating the healthcare system and community services
• Assessment and treatment of non-emergent, episodic concerns such as ear infections, cough, rashes

Translation services available upon request.

We Do Not Provide:

• Care for children 6 years of age and older
• Care for parents or adult family members
• Obstetrical/Prenatal Care
• Emergent/Urgent Care

Service Locations:

West Elgin Community Health Centre (West Lorne) - Wednesdays and Thursdays 8:30 a.m. - 4:30 p.m.

Southwest Public Health, St. Thomas Clinic - open Monday and Tuesdays 8:30 a.m. - 4:30 p.m.

Note that additional service location/hours may be offered in the future depending on community needs.

Self-Referrals and Referrals from Community Partners Accepted:

You will be contacted typically within 1-2 business days.
