Our Diabetes Care Team offers individual and group education with flexible hours at multiple locations in Elgin County. All adults living with diabetes or prediabetes, and those at risk for diabetes are invited to access these free services.

When it comes to managing your diabetes and maintaining health and wellbeing, our nurse and dietitian team is here to support you. We provide in-person and phone appointments and updates to your primary care provider. We also offer grocery store tours and classes on topics like healthy eating, lifestyle tips, and preventing diabetes complications. Programs vary by site. Our goal is to help you live well with diabetes through care that fits your lifestyle.
